Wrong file association

"Windows cannot open this file"
On Windows 10, this is expected without an extractor installed; see open a RAR file on Windows 10. On Windows 11, it can mean the archive uses a feature the built-in support doesn't cover, such as encrypted file names.
Missing context menu entries after an update
Windows updates occasionally reset or interfere with shell extension registrations. Reinstalling or repairing your archive program's installation typically restores the missing Extract Here or Add to archive... entries.
Blocked downloads and Mark of the Web
Files downloaded from the internet, including RAR archives, are tagged internally with a "Mark of the Web" flag. This can cause extracted executables to trigger extra warnings, or SmartScreen to block a downloaded archive's contents from running. Right-click the file, choose Properties, and if there's an Unblock checkbox near the bottom, you can clear it — but only for files you trust.
Path too long

Antivirus quarantine
Security software sometimes flags extracted files, particularly executables, and moves them to quarantine. Check your antivirus program's quarantine or history log if a file disappears right after extraction, and only restore it if you're confident the source was legitimate.
Permissions
Extracting into a protected system folder, or onto a drive where your account lacks write access, causes extraction to fail partway through. Extract to a location you know you have full permissions for, such as your user folder or Documents.
